In consideration of the gratuitous use of the KanOkla Networks headquarters facility for a fishing day (activity), to be held during the 2022 Fishing Season by KanOkla Networks, I, the undersigned, agree that:

1. KanOkla Networks shall not be liable or responsible for any injury or damage to:  

• my person or property or  

• the person or property of any other person or entity, if caused by, or attributable to, my person or property, and I hereby indemnify KanOkla Networks against, and hold KanOkla Networks harmless from, any such injury or damage.

2. I accept responsibility for any and all damage that may occur to KanOkla Networks’ facility during the activity, and will be responsible for repairing, or paying for the repair of, any such damage that may occur during the activity, provided such damage is caused by, or attributable to, my person or property; to the person or property of my ward(s) attending the activity; the person or property of my child(ren) attending the activity.

 

3. I am the parent or legal guardian of the child(ren) or ward(s) named to the right.

 

4. All references to KanOkla Networks include the respective members, shareholders, directors, officers, attorneys, accountants, agents, servants, contractors, subcontractors, representatives, successors and assigns thereof.

 

5. This agreement shall extend to and be binding upon me, as well as my heirs, devisees, legatees, trustees, and personal representatives.

 

6. Under penalty of perjury, I affirm that I have the authority to sign this agreement.
